血漿中の循環CXCR4とBMI-1 mRNAの組み合わせ：胃癌の潜在的な新規腫瘍マーカー

血漿中の細胞を含まない核酸は、新規腫瘍マーカーであると報告されています。しかし、胃癌におけるそれらの正確な重要性は不明です。本研究では、血漿中のCXCR4およびBMI-1 mRNAの循環に焦点を当て、胃癌患者の診断値を評価しました。CXCR4およびBMI-1 mRNAレベル、ならびにCEAおよびCA19-9タンパク質のレベルは、89人の胃癌患者の血漿で測定されました。これらのマーカーのレベルは、健康な被験者よりも癌患者の方が有意に高かった。しかし、手術後の発現が減少しました。循環CXCR4とBMI-1 mRNAの組み合わせの検出の感度は、CEAおよびCA19-9の検出の感受性よりも高かった。両方の遺伝子を発現する患者では、CXCR4およびBMI-1 mRNAレベルが強く相関していました。2つの循環遺伝子の発現もCEAレベルと強く相関しており、患者の年齢、組織学的型、臨床段階、細胞分化の程度、胃癌転移と有意に関連していました。我々の結果は、循環CXCR4とBMI-1 mRNAの組み合わせた発現が、胃癌の診断とモニタリングのために、CEAやCA19-9などの従来のマーカーよりも優れた、新しい腫瘍マーカーを表していることを示しています。

Cell-free nucleic acids in plasma have been reported to be a novel tumor marker. However, their exact significance in gastric cancer is unclear. In the present study, we focused on circulating CXCR4 and Bmi-1 mRNA in plasma and evaluated their diagnostic values for patients with gastric cancer. CXCR4 and Bmi-1 mRNA levels, as well as levels of CEA and CA19-9 proteins, were determined in the plasma of 89 gastric cancer patients. The levels of these markers were significantly higher in cancer patients than in healthy subjects; however, there was a decrease in their expression after surgery. The sensitivity of detection for the combination of circulating CXCR4 and Bmi-1 mRNA was higher than the sensitivities of detection for CEA and CA19-9. In patients expressing both genes, CXCR4 and Bmi-1 mRNA levels were strongly correlated. Expression of the two circulating genes was also strongly correlated with the CEA level, and was significantly associated with the age of the patient, histological type, clinical stage, extent of cell differentiation and gastric cancer metastasis. Our results indicate that the combined expression of circulating CXCR4 and Bmi-1 mRNA represents a novel tumor marker, superior to traditional markers such as CEA and CA19-9, for the diagnosis and monitoring of gastric cancer.
