低ナトリウム血症の診断と治療に関する臨床診療ガイドライン

血清ナトリウム濃度<135 mmol/Lとして定義される低ナトリウム血症は、臨床診療で遭遇する体液と電解質バランスの最も一般的な障害です。それは、微妙なものから重度、さらには生命脅迫まで、幅広い臨床症状につながる可能性があり、さまざまな状態を呈する患者の死亡率、罹患率、入院期間の増加に関連しています。それにもかかわらず、患者の管理には問題があります。大きく異なる状態での低ナトリウム血症の有病率と、多種多様なバックグラウンドを持つ臨床医によって低ナトリウム血症が管理されているという事実は、診断と治療に対する多様な機関および特殊ベースのアプローチを促進しました。一般的かつ全体的な見解を得るために、欧州の腎臓腎臓協会（ESICM）、欧州内分泌学会（ESE）、欧州腎協会（ERA -EDTA）、欧州腎臓のベストプラクティスに代表される欧州腎協会（ESE）、欧州腎協会 - （ERBP）は、低ナトリウム血症に自然な関心を持つ専門家を代表する3つの社会の合弁会社としての診断アプローチと低ナトリウム血症の治療に関する臨床診療ガイドラインを開発しました。方法論と評価への厳密なアプローチに加えて、私たちは、文書が患者の重要な結果に焦点を合わせ、日常の実践に関与する臨床医の効用を含むことを保証することに熱心でした。

Hyponatraemia, defined as a serum sodium concentration <135 mmol/l, is the most common disorder of body fluid and electrolyte balance encountered in clinical practice. It can lead to a wide spectrum of clinical symptoms, from subtle to severe or even life threatening, and is associated with increased mortality, morbidity and length of hospital stay in patients presenting with a range of conditions. Despite this, the management of patients remains problematic. The prevalence of hyponatraemia in widely different conditions and the fact that hyponatraemia is managed by clinicians with a broad variety of backgrounds have fostered diverse institution- and speciality-based approaches to diagnosis and treatment. To obtain a common and holistic view, the European Society of Intensive Care Medicine (ESICM), the European Society of Endocrinology (ESE) and the European Renal Association - European Dialysis and Transplant Association (ERA-EDTA), represented by European Renal Best Practice (ERBP), have developed the Clinical Practice Guideline on the diagnostic approach and treatment of hyponatraemia as a joint venture of three societies representing specialists with a natural interest in hyponatraemia. In addition to a rigorous approach to methodology and evaluation, we were keen to ensure that the document focused on patient-important outcomes and included utility for clinicians involved in everyday practice.
